Meeting with a R.D.N. (Registered Dietitian Nutritionist) for nutrition counseling is often covered by health insurance, which typically requires some initial information gathering over the phone or via email before the first appointment. Amber currently accepts the following health insurances:
· Blue Cross & Blue Shield
· United Health Care
· Medicare (typically only covered for individuals with diabetes and/or chronic kidney disease)
Make sure to verify your benefits with your insurance provider by calling the number on your insurance card. Consider asking the following questions:
· My diagnosis is _________. Is this covered under my plan?
· Is a referral required?
· How many visits are covered each year?
· Is there a co-pay or deductible due at the time of service?
If your current insurance plan does not cover nutrition counseling, or you prefer to pay out of pocket, contact Amber for her current rates. (Please note it's often more cost affordable to use one's health insurance, if you have coverage for nutrition counseling with a Registered Dietitian Nutritionist.)
Amber Yui reserves the right to charge a cancellation fee to individuals who fail to cancel their appointment with less than 24-hour notice. This policy applies to all initial and follow up appointments, for all new and returning clients.
